Chronic Care Management Program

A coordinated care strategy for individuals with several chronic illnesses is called chronic care management, or CCM. A committed care team collaborates closely with the patient to develop a customised treatment plan, offer continuing assistance, and help the patient communicate with other medical professionals. CCM services include care coordination, medication management, and routine phone or video check-ins. CCM aims to lower healthcare expenditures while simultaneously improving quality of life, lowering hospitalisations, and improving health outcomes for patients with chronic diseases. CCM services are frequently reimbursed by Medicare and other payers.
